В статье проведен анализ существующих методов лечения. В рамках клинического исследования рассмотрена результативность таких хирургических методов, как интерпозиционная артропластика, однополюсное и тотальное эндопротезирование, паллиативная хирургия. Установлено, что сроки иммобилизации и восстановления (4,2 и 7,6 недель) при данных методах неприемлемо велики. Доля пациентов с осложнениями составляет 23%, а восстановление подвижности сустава не достигается в 21% случаев. Субъективная оценка функциональности кисти по шкале DASH для всей когорты пациентов составила 9,7 баллов, а для третьей и четвертой стадий – 12 и 18 баллов соответственно, что свидетельствует о недостаточной эффективности применяемых методов. По результатам исследования авторы делают вывод о необходимости разработки нового хирургического метода к которому должны быть сформулированы требования, обеспечивающие эффективное оказание помощи пациентам данной категории.

The article analyzes the existing methods of treatment. In the clinical study, the effectiveness of such surgical methods as interposition arthroplasty, single-pole and total endoprosthetics, palliative surgery is considered. It has been defined that the periods of immobilization and recovery (4.2 and 7.6 weeks) under these methods are unacceptably high. The share of patients with complications is 23%, and recovery of joint mobility is not achieved in 21% of cases. Personal assessment of hand operation on the DASH scale for the entire cohort of patients was 9.7 points, and for the stages three and four – 12 and 18 points, respectively, which indicates insufficiency of the methods used. According to the results of the study, the authors conclude that it is necessary to develop a new surgical method to which requirements must be formed that would provide effective aid to such patients.
