Abstract
The study was conducted at the Rechitsa Central District Hospital from 2014 to 2016. Objective: to present the experience of applying a method for assessing the sensitivity of the cardiovascular system to intense single psychoemotional influences in order to facilitate its implementation in the clinical practice of cardiologists, therapists, and general practitioners. Objectives: 1) To examine the mechanisms of influence of mental factors on the strength of emotional reactions; 2) To determine clinically significant thresholds for deviations in hemodynamic parameters under the influence of emotional arousal; 3) To evaluate the distribution of impaired mechanisms of blood pressure regulation; 4) To compare the sensitivity and specificity of the test with clinical role-playing and an information test; 5) To describe the complications identified during the test.
Conclusions: 1) we established the mechanisms of influence of psychological factors on the strength of emotional reactions, which were described in linear regression models: OER = 3.42+0.22 A+0.20 D-0.18 E+0.21 F, NF = 0.21+0.27 STAI, NF = 0.58+0.11 NPA, NF = 0.631+0.006 VEI 2, C = 10.1-0.06 OEI, OER = 8.0-0.2 SP, OER = 4.6+0.6 AHR; 2) we determined the threshold values of the indicators: SBP ≥ 18 mm Hg, DP ≥ 1.135, SBP ≥ 10 mm Hg, PP ≥ 7 mm Hg. Art., VIC ≥ 19.0 units, DBP ≥ 4 mmHg, HR < 11 bpm; 3) three types of emotiogenic hypertensive reactions were found in patients with hypertension: 55 % were characterized by lower frequencies of reactions associated with the activation of the SAS and RAAS, 30 % were characterized by predominantly RAAS activation, and 15 % were characterized by simultaneous activation of the SAS and RAAS; 4) The sensitivity of the clinical role-playing test (23.9 %) was at least twice as high as the sensitivity of the information test (11.9 %) in relation to arterial hypertension; 5) two types of complications were found during the test: emotiogenic hypertensive reaction >180/110 mmHg in 15.2 %, strong evoked negative emotions of 9–10 points in 10.5 %.
