The problem of wound management infected and chronic wounds is still relevant in modern surgery. The number of nosocomial multidrug-resistant strains of bacteria is increasing, which reduces the significance of antibiotic therapy. One of the options for solving this problem is the development of new agents and methods of local treatment, in particular, in the creation of new highly effective wound dressings with antibacterial and reparative properties. Chitosan is a natural biopolymer that has its own antimicrobial activity, hemostatic and sorption effects, the ability to biodegrade and biostimulate repair processes. Due to the amino groups, chitosan can act as a carrier for antiseptic agents such as silver and cerium ions. The article presents the results of an experimental study that evaluated the antibacterial properties of chitosan nanofibers directly modified with silver and cerium ions against standard and hospital strains of microorganisms. The results obtained indicate a high antimicrobial activity of the studied samples and the need for their further study on the model of the wound process.
Aim of the study. To study clinical and radiological features of hammertoe deformity of the middle toes, accompanied by overload (primary) metatarsalgia.
The results of clinical and radiological examination of hammertoe deformity of 227 toes on 127 feet in 103 patients (96 women and 7 men) aged 31 to 79 years were studied. During clinical examination, the degree of transverse flatfoot and the variant of the 2nd, the 3rd, and the 4th hammertoe deformity were determined, the passive range of motion in the metatarsophalangeal (MTP) joints was measured, the Visual Analogue Scale (VAS) for pain and the American Orthopаedic Foot and Ankle Society Lesser Toe Scale (AOFAS-II-V) were used. X-ray evaluation determined the angular parameters of the forefoot and evaluated the ratio of the lengths of the metatarsals. The analysis showed that the deformity of the 2nd toe was noted in 54.62% of cases, the 3rd – in 32.16%, and the 4th – in 13.22% of cases. At the same time, the extensor variant of hammertoe deformity was observed in 29.9% of cases, the flexion variant – in 31.5%, and the extensor-flexion variant – in 38.6% of cases. X-ray showed a significant increase in the median values of the valgus angles of the 1st, the 2nd, the 3rd, and the 4th toes, as well as a significant excess of the length of the 2nd metatarsal over the 1st one. The median value of VAS reached 6 [6; 7] points, and the median AOFAS-II-V was only 35 [30; 47] points, while the state of the feet in 80.31% of cases was assessed as unsatisfactory. Thus, the deformity of the middle toes, complicated by overload metatarsalgia, is characterized by the prevalence of the most severe extensor-flexion variant of the hammertoe deformity, accompanied by an excessive length of the 2nd and the 3rd metatarsals, which leads to widespread corns on the sole and severe pain in the forefoot.
In some cases, in the depressive symptoms of schizophrenia, the phenomenon of mental pain or «vital melancholy» is traced, which often has an obsessive nature and proceeds with suicidal intentions. The low effect of existing methods of therapy of this condition with the help of psychopharmacotherapy is noted. In this study, the authors attempted to additionally influence this symptom complex using binaural stimulation of the brain, along with psychopharmacotherapy. The authors used The Psychache Scale to assess mental pain and measure it numerically, and the PANSS scale to diagnose the clinical symptoms of schizophrenia. In the main group of patients receiving binaural brain stimulation, it was possible to achieve better treatment results for a number of symptoms related to the negative cluster, the cluster of general symptoms and one more additional symptom: emotional lability than in the control group that did not receive this specific additional treatment. The use of binaural brain stimulation in combination with drug therapy has significantly reduced the severity of mental pain in patients with schizophrenia.

The aim is to evaluate the efficacy of the developed method of resection of the metatarsophalangeal joints (MPJ) in the surgical treatment of forefoot deformity in patients with rheumatoid arthritis (RA). The results of surgical treatment of the forefoot deformities in 34 patients with RA were studied. The operation was performed on both feet in 25 patients. A total of 59 feet were operated on. Resection of the heads of the II-V metatarsals using the traditional method was performed in group 1 (n = 28 feet) (the comparison group). In group 2 (n = 31 feet), resection of the heads of the II-V metatarsals was performed according to the method developed by us (the study group). The American Orthopedic Foot and Ankle Society Lesser Toe Scale (AOFAS-II-V) and the Visual Analog Scale (VAS) were used for clinical evaluation. Our own developed method of treatment (group 2) allowed to achieve better clinical and functional results according to the AOFAS-II-V scale and a more pronounced decrease in pain syndrome according to the VAS scale compared to group 1 where the traditional method was used.
The main technological possibilities of using Functional spectral-dynamic diagnostics (FSD diagnostics) for diagnostic support of tasks of the military medical service are considered. The main technological characteristics of FSD diagnostics are given.

Chronic venous insufficiency of the lower extremities is currently one of the most common pathologies of the human vascular system and represents an important socio-economic problem for developed countries both due to its high prevalence and due to the costs of diagnosis and treatment. In highly developed countries, 15–40% of the adult population suffer from chronic venous insufficiency, and decompensated forms with pronounced trophic skin disorders are detected in 15–18% of the population. Taking into account the high prevalence of chronic venous insufficiency among the population and a large number of people with decompensated forms of venous insufficiency, the treatment of such patients creates certain difficulties for the healthcare system and remains an unsolved problem both medical and socio-economic. In this work, we analyzed the approaches to the treatment of trophic disorders in venous insufficiency of the lower extremities that currently exist.
