Abstract

The health status is an important aspect of combat readiness and combat effectiveness of servicemen. The goal – to form an assessment of the health disorders military and epidemiological significance among conscripts in the Armed Forces of the Republic of Belarus. A random statistical analysis of medical reports (blank 3/MED) from military units was carried out, in these units more than 80% of the total number of servicemen in the Armed Forces of the Republic of Belarus served in 2003–2016. The International Classification of Diseases, the 10th revision (ICD-10) was used for unifying the recording of servicemen morbidity. When calculating the generalized assessment of the military and epidemiological significance of health disorders of conscripts the contribution to the structure of the class (group) of diseases was multiplied for death by coefficient 3, for dismissing – by 2, the remaining data – by 1. In the structure of the assessment of the disorders military and epidemiological significance 1st rank was composed by injuries, poisoning and certain other consequences of external causes (XIX chapter according to ICD-10), 2nd – diseases of the respiratory system (X chapter), 3rd – diseases of the digestive system (XI chapter), 4th – diseases of the circulatory system (IX chapter), 5th – diseases of the musculoskeletal system and connective tissue (XIII chapter). Contribution to the structure of general assessment of these classes was 73.2%. Among the nosologies (chapter groups) the 1st rank in the structure of the assessment of disorders military and epidemiological significance was acute respiratory infections of the upper respiratory tract (J00–J06 in the ICD-10), 2nd – injuries involving multiple body regions (T00–T09), 3rd – sequelae of injuries, of poisoning and of other consequences of external causes (T90–T98), 4th – diseases of oesophagus, stomach and duodenum (K20–K31), 5th – injuries to the head (S00–S09). The total contribution of these five nosologies to the structure of the general assessment was 37.4%.

Abstract

An analysis was made of the prevalence of stigmatization of people living with HIV in the Republic of Belarus following a survey of students at medical and pedagogical universities. The survey involved 100 respondents (including 27 men and 73 women; 50 of them – students of medical schools and 50 students of the Pedagogical University) were involved in our study. It was shown, that at present the stigmatization of people living with HIV remains a common phenomenon among the students of the Republic of Belarus, although it has a hidden nature. It turned out that the degree of stigmatization is lower for medical students with more complete knowledge at HIV-infection and for women.

Abstract

We studied medical reports (blank 3/MED) from military units, in these units more than 80% of the total number of servicemen in the Armed Forces (AF) of the Republic of Belarus served in 2003–2016. The International Classification of Diseases, the 10th revision was used for unifying the recording of servicemen morbidity. The average annual level of general morbidity among conscripts in the AF of the Republic of Belarus was (2860.7 ± 98.6) ‰, primary morbidity – (1638.2 ± 79.1) ‰, the need for dynamic dispensary observation – (93.5 ± 9.5) ‰, hospitalization – (931.6 ± 44.2) ‰, days away from work – (8406 ± 327) ‰, dismissing for health reasons – (15.80 ± 0.64) ‰, death rate – (35.1 ± 5.0) per 100 thousand of military conscripts per year. Leading nosologies (groups of classes) with percentage of 1% or more, which make significant contribution to the morbidity structure of conscripts of the AF of the Republic of Belarus have been revealed. It is necessary to develop an assessment of the military and epidemiological significance of health disorders of servicemen, which would integrate 7 summarized medical and statistical indicators of morbidity.

Abstract

The article presents the results of the study of the relationship between the risk of acute drug-induced extrapyramidal disorders in antipsychotic therapy of schizophrenia with the gene polymorphism of neurometabolism and the xenobiotic detoxification system: CYP2D6, DRD2, GST-M1, GST-T1, MDR1 and COMT. Found that the risk of acute drug-induced Parkinsonism linked to the carriage of allele «slow» metabolism of CYP2D6 * 4 (rs3892097) in the gene CYP2D6 and allele A2 increased gene expression level DRD2 polymorphic locus DRD2 / ANKK1 Taq1A (rs1800497) in ANKK1 gene, as well as the carriage T / T genotype of the MDR1 polymorphic locus (C3435T). Acute drug-induced akatisia found a connection with deletions in the genes GST-M1, GST-T1, which are responsible for the functioning of the glutathione xenobiotic detoxification system. The results of the polymorphism of these genes allows us to individualize the antipsychotic therapy of patients with paranoid schizophrenia, taking into account the risk of acute extrapyramidal disorders in them.

Abstract

The sensitivity of 136 Klebsiella pneumoniae clinical isolates collected in 2012–2015 years from hospitalized patients to antibiotics and phage therapy preparations was studied. 14 isolates with a hypermucoid phenotype (hvKP) and 122 isolates with a classical phenotype (cKP) were identified. A larger antibiotic and phage sensitivity of hypermucoid isolates in comparison with the classical ones is shown. Carbapenemase producers (OXA-48, NDM) were found only among isolates with a classical phenotype.

Abstract

The article analyzes the existing methods of treatment. In the clinical study, the effectiveness of such surgical methods as interposition arthroplasty, single-pole and total endoprosthetics, palliative surgery is considered. It has been defined that the periods of immobilization and recovery (4.2 and 7.6 weeks) under these methods are unacceptably high. The share of patients with complications is 23%, and recovery of joint mobility is not achieved in 21% of cases. Personal assessment of hand operation on the DASH scale for the entire cohort of patients was 9.7 points, and for the stages three and four – 12 and 18 points, respectively, which indicates insufficiency of the methods used. According to the results of the study, the authors conclude that it is necessary to develop a new surgical method to which requirements must be formed that would provide effective aid to such patients.

Abstract

46 patients with different functional classes of the heart failure (CHF) were examined. By means of the questionnaire and functional tests we made evaluation of the intensity of the fatigue syndrome to determine the physical status of the patients (handgrip test, SPPB-test, modified test with 6-minutes' walk). We were found out close relationship between the severity of the CHF and the intensity of fatigue, muscle strength, physical activity, tolerance to physical exertion. We used a pedometer to conduct the modified test with 6-minutes' walk.

Abstract

Mortality rates among people with schizophrenia spectrum disorders are approximately three times higher than among mentally health population of the same age. There seems to be a tendency towards increased mortality risks in patients with cardiovascular diseases, which may be a result either of cardiotoxicity or metabolic effects of antipsychotics.
To determine the correlation between intake duration, dosage of antipsychotics and development of metabolic syndrome we examined 30 female patients with diagnosed schizophrenia spectrum disorders, including their current state, markers of metabolic syndrome, as well as current and archival clinical records.
The study shows that development of metabolic syndrome directly depends on the intake duration, the amount of lifetime antipsychotics consumption, as well as the dosage of antipsychotics. The outpatient stage of treatment, as far as it seems, has greater influence on the patients’ metabolism.
This study is one of the first attempts to draw doctors' attention to preventing metabolic effects of the antipsychotic therapy, based on real clinical practice in Republic of Belarus.

Abstract

The article presents the results of our studies of the connection between neuropsychological and oculomotor parameters in schizophrenia and at a high risk of psychosis. It is shown that, at a high risk of psychosis, there is a separation of visual-spatial and auditory-speech perception. The authors come to the conclusion that a decrease in the inhibitory influence of the parietal parts of the brain on the process of the appearance of saccades leads to a decrease in the duration of their latency. With a high probability, a decrease in the latency of saccades should be considered a psychophysiological marker of high risk of psychosis.

Abstract

The spatio-temporal dementia model is described, that represents the process of leveling the spatial structure inherent in healthy people due to the formation of empty loci inside it, the transition of the spiral movement of individual time into the rectilinear directed backward until the point position is reached. These fundamental changes lead to the patients consciousness and its controlled cognitive and mental functions fading.

Abstract

The characteristics of Lyme disease epidemic process were specified in а large industrial city since the introduction of the official registration. А continuous, retrospective, longitudinal epidemiological study were carried out. Differences in the characteristics of the long-term dynamics of the epidemic process of Lyme disease in 1994–2002, 2003–2008, and 2009–2017, the dynamics of the population's accessibility to tick bites in Minsk in 2003–2017, outpatients visitors attendance, the long-term trend were established. A comparison of data on the population's appealability to tick bites, data on bacterial infectivity of ticks, and the incidence rate of the population of Minsk were carried out. The relative risk of being bitten by ticks in different years, prognostic number of persons attacked by ticks in Minsk in a year were introduced. The new scientific data testify to the persistence of a high risk of human infection with pathogens of Lyme disease.

Abstract

According to the results of monthly distribution of hospitalized for two years 1699 patients with burns it was found that from may to August, their share increased 1.5–2.0 times, as compared to the calendar seasons (winter, spring, autumn), 2.1–2.8 times due to more frequent contacts with year-round current traumatic agents (hot liquids, flames, hot solid objects, electric current), and also as a result of increased sunlight exposure to skin to and the emergence of aggressive chemical compounds of plant origin. Despite the increase of the number of victims in may-August, among them the proportion of people with extensive burns (21 % and more) was significantly lower (8.6 %) than in winter (24.4 %) and autumn (17.0 %), and with the destruction of tissues of IIIB and IV degrees decreased, respectively, from 10.0–11.0 % to 7.3 % and from 19.7–25.8 % to 13.4 %, which was accompanied by a decrease in the frequency of bacterial complications such as pneumonia, septicemia, bacteriuria.

Abstract

The pericarditis remains the most common pericardial pathology in clinical practice. A variety of forms and clinical manifestations, often less pronounced or masked by the symptoms of comorbidities, determine the hypodiagnosis of pericarditis.
The European guidelines for the diagnosis and management of pericarditis was updated in 2015 and giving certain practical algorithms.
The article presents both new recommendations and classic ideas about clinical diagnostics, which do not lose their positions. The terminological definitions, approaches to classification are systematized; the treatment regimens of individual etiological forms of pericarditis are given.
