The aim: to evaluate the results of surgical treatment of patients with fractures of the zygomaticorbital complex after reposition and osteosynthesis with fixation at one point along the zygomaticfrontal suture.
The object of the study was 12 patients (10 men, 2 women) with fractures of the zygomaticorbital complex undergoing treatment in the Department of Maxillofacial Surgery No. 11 of the City Clinical Hospital in Minsk for the period from December 2018 to June 2019.
Materials and methods: The patients underwent surgical treatment – reposition and osteosynthesis of the zygomatic-orbital complex with fixation of the fracture along the zygomaticfrontal suture. Surgical access to the fixation point was through the upper eyelid. The results of surgical treatment were evaluated based on a comparative analysis of the clinical picture of patients before surgery and 1 month after surgical treatment. The following clinical symptoms were analyzed: pain in the fracture area; the level of sensitivity loss in the innervation zone of the infraorbital nerve; the presence of bone steps in the area of fractures; deformation of the middle zone of the face; degree of restriction of mouth opening. Manifestations of clinical symptoms were recorded according to our proposed scale from 0 to 2, depending on their severity.
The standing of the fragments after the operation was evaluated by con-beam computed tomography scan. The Wilcoxon test was used to determine the statistical significance of the study results.
Results: 1 patient did not appear for examination one month after surgical treatment (8.3% of the entire sample). Pain in the fracture area, limitation of mouth opening and deformation of the middle zone of the face were absent in all patients who came for examination 1 month after surgery (91.7% of all operated patients). In 6 patients (50.0%) there was a moderate decrease in sensitivity in the innervation zone of the infraorbital nerve, positive dynamics compared with the state before surgical treatment. 5 patients (41.7%) had a complete recovery of sensitivity. 3 patients (25.0%) had a moderate bone step in one of the fracture lines. In 8 (66.7%) patients, bone steps were not determined. When analyzing the studied signs in the group by the Wilcoxon criterion, there were statistically significant differences in the following signs: pain in the fracture area (z = 2.9341; p = 0.0033); the level of sensitivity loss in the innervation zone of the infraorbital nerve (z = 2.9341; p = 0.0033); the presence of bone steps in the area of fractures (z = 2.9341; p = 0.0033); deformation of the middle zone of the face (z = 2.9341; p = 0.0033); degree of restriction of mouth opening (z = 2.5205; p = 0.0117).
Conclusion: Surgical treatment of fractures of the zygomatic-orbital complexes with fixation at one point along the zygomatic-frontal suture showed a positive dynamics in the condition of patients 1 month after surgery. All patients who came to the examination noted the absence or decrease in the degree of manifestation of symptoms present before surgical treatment. This fact indicates the advisability of carrying out the surgical treatment of fractures of the zygomatic-orbital complex with fixation at one point along the zygomatic-frontal suture.

Conclusion. The results obtained made it possible to clarify the characterological features of the reflected viscero-vertebral pain syndrome in cervical, thoracic and lumbar degenerative disk disease, viewed from the perspective of a disease of the whole organism.
We conducted a selective statistical analysis of medical reports on the health status of personnel in form 3/MED of military units, in which at least 80% of the total number of military personnel recruited by the Armed Forces of the Republic of Belarus for 10 years (2009–2018) served. The average annual level of the general incidence of conscripts with mental disorders amounted to (30.1 ± 1.8) ‰, the primary incidence (16.1 ± 1.2) ‰, the need for follow-up (5.1 ± 0.6) ‰, hospitalization (13.8 ± 0.5) ‰, days of labor loss (283 ± 11) ‰, dismissing (5.38 ± 0.40) ‰. Polynomial trends with different determination coefficients showed tendency to decrease in the last observation period in almost all types of medical and statistical morbidity. The need for follow-up showed a trend of increasing data. The constructed assessment of the military-epidemiological significance of mental disorders for conscripts with high proportions included neurotic, stress-related, and somatoform disorders (group 5, F40 – F48 according to ICD-10) and personality and behavior disorders in adulthood (7th group, F60 – F69), mood disorders (affective disorders) (4th group, F30 – F39), unspecified mental disorders (11th group, F99). It can be assumed that the quality of the draft contingent is important in the development of the incidence of mental disorders in conscripts of the Republic of Belarus, and military-professional factors are not leading and may act as conditions for development of mental disorders.
The article presents the results of the effect of hypoxia on the peritoneum. The sample included 10 patients in whom the partial oxygen pressure in the adhesion tissue was recorded intraoperatively by direct oximetry, followed by an immunohistochemical study of HIF-1α expression in it. The study revealed: tissue hypoxia promotes the expression of HIF-1α, affects the development of fibroblasts and, as a result, the production of coarse fibrous tissue in the abdominal cavity. A decrease in hypoxia in the area of surgical intervention will obviously reduce the likelihood of an adhesion process in the abdominal cavity.
The effect of alexithymia, the development of abstract thinking and the state of emotional intelligence on the reactivity of emotions of four modalities: “anger”, “ fear”, “sadness” and “ joy” were found in 35 patients with a cardiological profile. The presence of signs of alexithymia, an outwardly oriented type of thinking, ability to work and sensitivity enhances, developed abstract thinking and expresses emotions in the modalities “anger”, “ fear”, “sadness”, “ joy” and emotional reactivity in general. A high level of general, intrapersonal, emotional intelligence, the ability to understand emotions reduce the expression of emotions “Fear”.
Group B Streptococcus (GBS) is the most common etiological pathogens of bacterial neuroinfection in children younger than 3 month old.
The median age of 58 patients in our study was 19 (interquartile range: 14–36) days old. The diagnosis of neuroinfection was made in 2 cases (3,4%) on the prehospital phase. The most popular complains were fever (92,8%), loss of appetite (53,6%), irritability (51,8%). In 5,4% of cases the temperature at the onset of disease were normal, in 1 case (1,8%) it wasn’t measured before hospital admission. On admission 26,8% of patients had hemodynamic instability. The incidence of hyperesthesia (84,4%), stiff neck and archet back (53,1%), positive Lesage symptom (90,6%) during hospitalization appeared significantly higher in children being late neonatal period compared to infants (p < 0,05). Infants had toxic shock syndrome significantly higher compared to children being late neonatal period (F = 0,01, р = 0,013).
Neuroinfections causing by group B Streptococcus were accompanied by anemia, leukopenia and cytosis in the CSF with neutrophil predominance.
At the hospital discharge 50% of children being late neonatal period and 70,5% of infants had adverse consequences of infection.
Thus, neuroinfection causing by GBS in children younger than 1 year old is a severe infection which leaded to adverse consequences in more than half patients of our study. Every 5th child in our sample had toxic shock syndrome. The problem with diagnosis infection in children younger than 1 year old associated with nonspecific clinical features of neuroinfection. The most popular clinical features at the onset of neuroinfection causing by GBS were fever (92,8%), bulging of fontanelle (62,5%), high pitched cry (60,7%) and positive Lesage symptom (51,8%). The absent of fever at the onset of disease in younger than 1 year old shouldn’t except neuroinfection.
The purpose of this work: to study and analysis of quantitative and cooperative relationships of cells of the immunocompetent system in the thymus gland and spleen in acute respiratory viral infections (ARVI), taking into account the type of pathogen. A study of autopsy material during acute respiratory viral infections revealed changes in cell cooperation in the accumulation and distribution of effector cells of the broncho-vascular barrier. In the study of the thymus gland and spleen, a change was found in the cooperative relations of cells of the immunocompetent system in acute respiratory viral infections, interorgan analysis showed a difference in the relationship between the effector cells of the broncho-vascular barrier and the cells of the immune system of the organs of the central and peripheral link of immunity. This will allow using system positions to assess changes in the immune system of the child‘s body during acute respiratory viral infections.
Objective: development of an algorithm for diagnosing and choosing the tactics of surgical intervention for localized kidney cancer based on the analysis of multispiral computed tomography data (MSCT) to reduce the risk of perioperative complications and to improve the functional results of nephron-sparing surgery (NSS).
Subjects and methods. As material for prospective randomized clinical trial were results of MSCT and surgical treatment of patients (n = 130) with clinically localized kidney cancer at an early stage of a disease (40 (30.8%) patients with a stage of T1aN0M0 and 90 (69.2%) patients with T1bN0M0 stage) which were operated in our clinical center during the period from 2014 to 2019 years. Assessment of the surgical complications was carried out on classification of Clavien-Dindo. Calculation of a glomerular filtration rate (SKF, ml/min./1.73sq.m) was carried out on the equation of MDRD (Modification of Diet in Renal Disease). All diagnostic tests in the oncological clinic were performed on the X-ray computer tomography, based on the operating system «Windows» (General Electric (GE Healthcare) company, model CT660, 64-spiral system with IQ Enhance technology).
Results. According to the results of monovariant and multivariate analysis, it was found that of all the studied parameters, only nephrometric MSCT data, such as the diameter, the spatial localization of the tumor in the kidney, the presence of a pseudocapsule, the predominance of a solid component at the periphery of formation, the presence of an extrarenal component in the tumor, as well as deformation of the pyelocaliceal system (р < 0,05 for all indicators), are the main differential diagnostic criteria justify the possibility of the technical enucleoresection or classical partial nephrectomy. In the comparative analysis of results of PNS it is revealed that use of the developed way of nephrometry assessment of a tumor of a kidney according to MSCT had statistically significant impact on duration of surgical intervention (U = 53,0, р = 0,001), blood loss volume (U = 226,5, р = 0,001), time of ischemia (F = 0,1, р = 0,008), and also on functional conditions of renal function after operation (U = 354,5, р = 0,035). According to the results of a morphological study of surgical material, no relationship was found between the indentation from the tumor node for the progression of the disease (F = 1,0, p = 1,0).
Conclusion. Development and use of a new nephrometry score of assessment certain differential and diagnostic criteria of a tumor according to MSСT is a basis for the choice of surgical intervention (nephron-sparing surgery or enucleoresection) at the localized kidney cancer form.
The approach to the study of dissociative disorders from the point of view of their spatio-temporal organization made it possible to study the fundamental laws of their formation. It is shown that the latter consist of alternating processes of dissociation and association of individual spatio-temporal models of patients, where the first leads to the decay of the model, and the second – to its reproduction. However, the association with these disorders is not complete, since it occurs by copying the spatiotemporal models of other people, as a result of which other people‘s mental functioning programs are introduced into the consciousness of patients. The features of these processes in patients with hysterical personality disorder, actually dissociative and conversion disorders are described.

The problems of environmental pollution by unused medical waste of medical institutions and medicinal waste from the population due to the unnecessary, unsuitable, expired pharmaceutical preparations and related chemotherapeutic agents are considered. The bioecological consequences of the penetration of medicinal products and their metabolic substances into soil, aero and hydroenvironments, including ground and underground waters, and then, in plants, fish, domestic birds and animals were revealed. It is shown that one of the most dangerous channels of the secondary circuit of medicinal principles, which have a serious uncontrolled quasi-therapeutic effect on humans, is food chains of the form «pharmaco-infected water – plant food – people», «pharmacoinfected water – plants – animals – meat dairy products – people», «pharmacologically infected fish – people», as well as channels for direct transmission of drug impurities directly under the scheme «pharmacologically infected water – water treatment system – people». A set of mechanisms and measures aimed at reducing the concentration of pharmaceuticals and their metabolites in the environment and increasing the level of its environmental safety is proposed.

HISTORY OF MILITARY MEDICINE
CHRONICLE
The 5th International Military Medical Forum «The Great Wall» was held in the People’s Republic of China on March 23–26, 2018, in which representatives of the Armed Forces of the Republic of Belarus took part. Own data of a nationwide population-based observational study evaluating the true prevalence of mitral valve prolapse (MVP) among young people across the Republic of Belarus were disseminated («global disease burden») were presented. The fact of an increase in the frequency of the MVP was indicated in the Republic – up to 7.9%, compared with the generally accepted population in Europe – 2.3%. So, MVP had held a leading in the structure of cardiovascular pathology among young men of conscription age (18–27 years old) and this had led to release from conscription.
