Abstract

Introduction One of the most frequent and dangerous complications of destructive pancreatitis is the affect of the retroperitoneal parapancreatic cellular tissue. The term parapancreatitis is often used to define it. Researchers distinguish four main forms of parapancreatitis: serous, serohemorrhagic, infiltrative-necrotic and pussy-putride.

Goal. To reveal morphostructural changes in the pancreas and target organs most characteristic for various forms of parapancreatitis.

Materials and methods. Study conducted retrospective analysis of 203 autopsy protocols of patients who died from acute pancreatitis (K.85) and chronic pancreatitis with outcome in pancreatic necrosis (K.86), who were treated in the surgical departments and intensive care and resuscitation departments of the Minsk city for 2015–2019.

Results. In our study, there were 2 forms of parapancreatitis: serohemorrhagic in 18 (8.9%) cases and infiltrative-necrotic in 185 (91.1%) cases. Study examined the structure of complications of destructive pancreatitis, combined with various forms of parapancreatitis. Study examined morphostructural changes in target organs (liver, kidneys, heart, lungs) for various forms of parapancreatitis.

Conclusions. Infiltrative-necrotic parapancreatitis occurred at a latest stages of the disease than serohemorrhagic. Fibrinopurulent peritonitis and multiple organ failure are statistically significantly more common complications in infiltrative-necrotic parapancreatitis in comparison with serohemorrhagic. The rate of neutrophilic infiltration of the hepatic lobules in the liver tissue is statistically significantly higher for infiltrative-necrotic parapancreatitis in comparison with serohemorrhagic, and frequency of fibrotic and atrophic changes in the pancreatic tissue is higher. For other morphostructural changes in the liver tissue and also in the tissues of the kidneys, lungs and myocardium there were no differences between different forms of parapancreatitis.

Abstract

Markers of the biochemical state of alcohol dependence can give clinicians objective data on the consequences of drinking alcohol or contribute to optimal treatment control. The article presents data from the study. The purpose of the study was to evaluate the effectiveness of the use of methods of normoxic therapeutic compression (NТC) and hyperbaric oxygenation (HBO) in combination with standardized drug therapy (SDT) in stopping alcohol withdrawal syndrome based on biological markers of alcohol intoxication. Three groups of patients in a state of moderate alcohol withdrawal were examined. Two comparison groups received SDT in combination with NТC and HBO, one group only SDT. The general biological markers of alcohol intoxication were investigated. Some specific tendencies of influence on the dynamics of the levels of biochemical markers of alcoholization in alcohol withdrawal syndrome against the background of the use of oxygen therapy methods in the complex treatment of alcohol withdrawal syndrome are identified, compared with standardized methods of pharmacotherapy.

Abstract

Arterial hypertension (AH) is one of the main risk factors for the development of cerebrovascular diseases (CVD), which in turn are one of the most important medical and social problems in the world and in our country. This is due to the severity of these diseases with temporary and permanent disability, a significant proportion of mortality from them in the structure of diseases of the circulatory system and total mortality from all causes. Long-existing asymptomatic hypertension leads to a slow progressing brain damage, with the manifestation of encephalopathy and with an increase in pathological foci (lacunar infarctions) in the white matter leads to the development of vascular dementia. The most complete and accurate picture of the presence and severity of brain damage can be obtained using neuroimaging – MRI of the brain.

Abstract

Otorhinolaryngology is always a topical issue effect of local processes of different nature (both inflectional or non-infectional) the overall reactivity of the organism, the most revealing reaction of the immune system. There are a number of biologically active substances, the change of concentration which reflects changes in the internal environment of a macroorganism in response to the manifestations of inflammatory or allergic reactions, including local character. One of the factors of humoral immunity are beta-lysine. We’ve studied the concentrations of beta-lysine in patients with chronic compensated tonsillitis and perennial allergic rhinitis. It is established that in chronic tonsillitis in remission phase significantly reduced the content of beta-lysine to 12.3±1.4% of that can be regarded as a risk factor for the development metatonsillaris complications. In patients with persistent allergic rhinitis the level of beta-lysine is correlated with the activity of the immune response to allergens.

Abstract

Sleep disorders are common to many mental disorders, including depression. The aim of the study was to evaluate the effect of exercise on sleep quality in the complex treatment of depressive disorders. The exercise program included cardio, strength, endurance, flexibility exercises and was conducted under the guidance of a physiotherapy instructor. Sleep quality was evaluated at the beginning and at the end of the inpatient phase of treatment. The severity of sleep disturbances was assessed using the Pittsburgh Sleep Quality Index (PSQI) methodology. Two groups of patients were formed: (1) the main group - patients engaged in physical exercises according to the appropriate program in addition to standard treatment (n = 107, average age 41.88±12.66 years); (2) comparison group – patients receiving standard treatment in accordance with the Protocol of Diagnosis and Treatment (n = 100, average age 42.54±13.63 years). In both groups, a significant reduction in sleep problems occurred during treatment. At the end of the inpatient phase of treatment, the differences between the groups were significant for a number of indicators of disturbed sleep (associated with breathing, snoring, temperature discomfort) with the best results in the main group. Exercise has contributed to a better solution to sleep problems in patients with depression than standard treatment with pharmacotherapy.

Abstract

The authors studied the long-term results of treatment of 102 patients with degenerative injuries of the rotator-bicepital complex on the basis of the 3rd traumatology and orthopedic department of the city clinical hospital No. 6 of Minsk from 2012 to 2017 using widely accepted approaches based on the introduction of hormonal drugs. Patients were divided into two groups, in the first group; the drug was administered under ultrasound guidance, in the second group by anatomical orientation. Controlled drug administration significantly improved the results of targeted drug administration and achieved more favorable treatment outcomes in patients with damage to the rotator-bicepital complex, when evaluated according to the OSS scale, the average value of the indicator before treatment was 55 [44; 57], after treatment – 16 [12; 22] and on the UCLA Shoulder Scale, before treatment with Me [Q; Q] – 16 [8; 23], after treatment with Me [Q; Q] – 37 [30; 38]. A large number of adverse results on the OSS scale – n = 8 (17.4%) and on the UCLA Shoulder scale – n = 14 (30.4%) make us think about improving the existing tactical approaches to the treatment of partial degenerative injuries of the rotator-bicipital complex.

Abstract

A study of 27,429 specimens was conducted in an outpatient population of patients, including men and women who applied for examination for sexually transmitted infections. Positive results for the detection of the pathogen M. hominis were obtained in 5.1%, of which the proportion of mono-infection was 0.3%, combined with U. urealyticum – 4.8%. During the 9-year follow-up, there was a statistically significant tendency to reduction of the detection of M. hominis infection among this category of patients. The detection rate of «pathological» contamination (> 104 CFU/ml) was 6.3% in case of mono-infection, in combination with U. urealyticum 9.1%. There were no statistically significant differences indicating a mutual aggravating effect of the combination of two Mycoplasma pathogens (p > 0.05). Tetracycline antibiotics and fluoroquinolones, which have shown the best results of laboratory testing, should be considered the drugs of choice in the treatment of M. hominis infection. For the treatment of pregnant women and children, josamycin can be recommended. Treatment is indicated mainly for patients with clinical signs of inflammation.

Abstract

The aim of the study was to determine the methodology for calculating the proper vital capacity of the lungs, the most suitable for use in conducting in-depth medical examinations of conscripts. For its implementation, an anthropometric examination of 647 military personnel was carried out, including determination of height, body weight, chest circumference, indicators of lung vital capacity were evaluated, and the relationship between them was studied. The most commonly used methods for determining the proper vital capacity of the lungs are analyzed. As a result, it was found that the magnitude of the vital capacity of the lungs is affected by growth, body weight and, to the greatest extent, the size of the chest circumference of the examined. However, it does not depend on age. In the analyzed methods for determining the proper vital capacity of the lungs, age indicators are used, which in the category of patients examined by us do not affect the lung capacity and do not take into account the size of the chest circumference. Based on this, we developed our own method for determining the proper lung capacity, based on determining the height, body weight and chest circumference of the subjects and allowing, in our opinion, to more adequately assess the functional state of the respiratory system of military and plan and organize based on the data obtained measures to preserve and enhance their health.

Abstract

The purpose of this study was to study the indicators of physical development and physical fitness among cadets of the military medical faculty and to determine the relationships between them. Physical development was assessed by indicators of growth, body mass index, chest circumference, vital and strength indices. Physical fitness was determined on the basis of the results of pulling up on the crossbar, running 100 meters and 1000 meters. As a result, it was found that the organization of the educational process and daily activities at the military medical faculty contribute to an increase in physical development among cadets. The existing relationship between indicators of physical development and physical fitness should be taken into account when organizing and conducting activities to strengthen the health of this category of military and increase their level of physical fitness.

Abstract

Bone defects in the jaw that remain after removal of foci of chronic odontogenic infection, cysts, benign neoplasms, sequesters, reduce the strength of the bone structure, worsen the functioning of the teeth, lead to a violation of the chewing act, noticeably prolong the recovery time of the alveolar processes, complicate, and often make impossible functional prosthetics.

Aim of the study – analysis of the long-term results of the use of calcium–phosphate ceramics in outpatient surgical practice.

Objects and methods. Clinical studies were conducted on 174 patients operated on with the use of calcium phosphate ceramics «KAFAM» for tumor-like formations, root cysts, and retained teeth. When planning surgical intervention, we individually selected the desired form of implantation material for each patient, depending on the shape and size of the defect.

Results. Analysis of the long-term results of the effectiveness of the use of calcium–phosphate ceramics «KAFAM» was objectively evaluated according to the data of radiation research methods. Long-term results were traced in 42 (24.1%) of 174 patients. Comparative X-ray examination of patients with bone postoperative defects showed complete filling of the defects with bone regenerate. In the long-term, in patients at the site of the former defects, mature bone tissue was determined radiographically, which did not visually differ from the surrounding bone tissue.

Conclusion. The use of bioactive bone-replacing material of calcium phosphate ceramics in surgical treatment allows you to fill in bone defects in the alveolar process of the jaw, stabilize and maximize the functionality of the teeth in the field of surgical intervention and thereby increase the effectiveness of complex treatment.

Abstract

Practical activity of a specialist in any industry creates conditions for the emergence of interesting, non-standard cases, which add to the «treasury» of his experience, and allow us to suggest possible options for action in similar situations. The history of forensic medicine (forensic medical examination) is the history of human destinies and relationships that, intentionally or by chance, often have an unpredictable or fatal outcome. There is not a single section of forensic medicine in which the study of medical and biological factors (objects) found at the scene of a crime or accident would not occur cususes that are interesting in themselves and instructive in terms of preventing the consequences of socially dangerous or careless acts, as well as foreseeing the fatal confluence of any circumstances. The article has a specific practical interest and makes you be careful when handling traumatic items.
